I'm trying to add a HA05 Screw-in Lamp Module to my ISY994i with Firmware v.4.5.4.  

I can successfully Remove/Exclude the Z-Wave device but am unable to Add/Include the module, as if the ISY994i just doesn't see it.  I've even tried to Add/include the device while the module and ISY were less than 2 inches apart.
