"Cannot Determine Insteon Engine"

I have a conventional 3-way circuit, both sides controlled by an Insteon on-off switch. I use a Universal Devices ISY994 for automation control. Everything worked fine (for years) until both switches suddenly appeared as inoperative on ISY994.  Universal Devices says it's an Insteon problem so, thinking the switches must have gone bad, I replaced one of them but, when I try to add that new switch to ISY994, I still get "Device Cannot be Added - cannot determine Insteon Engine" message (the same message I was getting with the old switches).  I'm stumped.  Any advice out there?

Will try that - assumed a brand new switch would be clean of junk but who knows?  Thanks!

They should be... every QA process I've ever seen has a final step that clears out all the QA data and test stuff, and returns the device to a known standard shippable state.  All except, it seems, whatever supplier SmartHome uses to QA their products!  I've certainly seen them show up with random addresses in the links table, and I've seen a KPL arrive with at least one button in a "Toggle-On" mode.  Others have reported devices in undocumented states, although the more common problem is that they arrive as new but are set to respond to an X10 address.

Rule-of-thumb: always, always set a new device up on your bench (with an extension cord, etc), factory-reset it, and test it -- before you go through the trouble of installing it.

After all the agony - tried the reset, still didn't work, revisited the wiring and I must have changed a wire from the proper set-up when I added the new switch.  I guess ISY994 was just trying to tell me I'm an idiot!  Thanks for your help, anyway!

Excellent advice - I have a ton of Insteon devices around the house and that advice would have saved me a LOT of work.  I'll see if this old dog can learn a new trick.  Thanks again.

Quality Assurance is not Quality Control and means they don't control quality!

They assure YOU of the quality and will take action if YOU find something wrong.
