ZWave device not showing all nodes

I added a new recessed door sensor (zwave plus) from Monoprice and while I'm able to add it, I only get the notify sensor and sometimes the tamper sensor node in the ISY admin tool. Looking at the log from ISY, it's seeing and receiving status on door openings and closings, but it doesn't disclose this in a node. See entries in log here where "ZW026_1 Reports value" is sent whenever I trigger the reed sensor:

I'm running ISY 994 Pro with v4.7.3, ZWave 4.55 and bootloader 1.03.

I've tried removing/adding 10+ times, I tried the "syncronize all" and "synchronize new and deleted". I've done query, I've removed the node without exclusion (ie, select trashcan) and then synced. I forced removed it, I factory reset the sensor, I tried "Repair links" and "Heal network" but at best, all I got was the tamper sensor node.

Any suggestions?
