Ecolink TILT-ZWAVE2.5-ECO fails to add

Trying to add Z-Wave tilt sensor, specifically "Ecolink TILT-ZWAVE2.5-ECO"

 

Using 994i with Z module

ISY Firmware version 5.0.16C
Z-Wave version: 6.81.00

Tried the following with cover removed:
1. Set to exclude in ISY, "add battery", the ISY stopped listening
2. removed battery
3. Set to add mode in ISY
4. Insert battery, stopped listening, included log below

Anybody have success?

Did you ever figure this out?  I'm not sure if I'm having the same issue yet, but I can't seem to add two z-wave devices, different brands and one is a new ecolink window/door sensor.  They never show up.  I haven't seen anything useful in the logs yet -- trying to generate that now.  I'm running the same versions you are.
