Scenes not always 100% reliable but no errors

I'm seeing some odd behaviors where a scene, for example, kitchen with ~5 devices and a total of 12 members (due dual 8 keypad for easy control) isn't propagating but also not generating an error in the Admin panel.

For example,

I have a movie mode which, among things, turns off the kitchen scene. When this happens, quite often one of the controllers in a separate room isn't updating. It still shows the scene as ON when it clearly isn't. But I'm not seeing the typical "can't communicate with device X".

What could cause this?

It can be a noise issue or a bad link. Try doing a restore device on the KPL.

Done it several times. Seems like it's more likely to happen if there is a lot of insteon traffic on my network. I thought it would confirm reception of command?

A Scene initiated by a Program (PLM) is not ACKed by device so there is no retry.

This can result in a device being being out of sync if network issues.
