New ISY994i ZW+PRO problem after factory reset

New ISY994i ZW+PRO. Before reading the user guide I was able to add/include 2 GE Z-Wave light switch. I did a "factory reset Zwave Dongle" after reading the user guides which removed the 2 GE Z-Wave device and now not able to add/include. Also I did a "Upgrade Z-Wave firmware" per user guide, which failed. Currently Z-Wave version 6.81.00. I tried "remove/exclude Z_Wave device" which did not work. Please help a new member.
