Ok, update... I now have 2 zwave plus modules... Evalogik dimmer module and a magnetic door sensor.  I first put isy in add mode, then the dimmer (triple click, button flashes).  Event viewer shows listening for modules to add.  Dimmer actually stops flashing as if add is complete.  Isy console has no new events and progress bar shows zero progress.

 

Update... Looks like these devices stay in add mode for 30 sec then stop... So I don't think module thinks it has been added.

Any ideas?
