ZWave not working

Any idea why my zwave part of my ISY will not detect anything?  I have tried reseting the controller as well as updating the firmware.  The controller is integrated with my ISY.

Firmware - Insteon_UD994 v.5.0.13D

UI - Insteon_UD994 v..0.13D

I am trying to add a zwave water sensor and a zwave smart outlet.  Both had worked in the past then stopped working.  The water sensor has new batteries and the smart outlet turns on and off using the button on it.

Have you tried excluding them first before including them?

Even with new devices sometimes they don't get excluded from the factory QA controllers and they will not include into another controller.

Yes, these devices had worked at one time.  I can get ISY to see them to add them some times but then I get an error saying failure to connect to device.  I am thinking it is not a device issue as I get the same error with 3 different ZWave devices.  Could the ZWave controller in the ISY have gone bad?  If so is that replaceable or do I need to replace the whole box, or can I just get a ZWave dongle for it?

From my understanding the z-wave dongle is replaceable which makes things nice for if it needs replacement or for upgrading.  

What kind of errors are you getting?  I just deployed a bunch of z-wave devices yesterday and I also have received communication errors on several of the devices that I did not receive on my other controller.  I'm not sure though if these errors are device related or ISY related or possibly mesh issues as some of the devices are at further distance.

I updated they Firmware on my ISY the other day to v.5.0.13D.

After I did that, all the Z-Wave devices disappeared.  As I turned them one and off one at a time, they would show up on the NETWORK device list.  I changed the names to what they used to be.  I then looked at the Programs.  All Z Wave devices in any program disappeared.  I was able to communicate with the Z-Wave devices and send On-Off commands.

The next day, I looked at the Programs again.  All the missing Z-Wave names were back.  I tried communicating with the devices.  Only 5 out of the 10 devices respond.

What is the best way to proceed?  Do I have to nuke the unresponsive devices and re-install them?  What tricks are available?  I have done the Repair Links command a few times plus the HEAL Z-Wave network.

I did read the upgrade instructions before doing anything.  I even thought I was saving a database that would contain everything.  I have no other Z-Wave controller to transfer control to.  I was under the notion that with only  12 Z-Wave devices (all Leviton Z-Wave switches and Dimmers), if I screwed it up, I could always re-enroll them.  I can see Hail messages in the event viewer from the devices that no longer respond to the ISY when I go up to them and turn them on and off.  I suppose that I will have to delete them and re-enroll them.  What is the best way to do that?  I saved the Event Viewer log file.  If you think there is something to be gained by you taking a peek at it I attached it to this reply.
