Insteon Smart On/Off Plug-in Module, 2635-222 randomly comes on

Any ideas why my Insteon Smart On/Off Plug-in Module, 2635-222 randomly turns on "maybe" once a month.. i'm going to start creating a log when the event happens but i'm struggling to figure it out.   Unfortunately I didn't have logging level 3 enabled so I can't see what happened during night / early am that may have triggered it to turn on.

I have done a factory reset on the device twice now to ensure it was wiped.  The challenge is it maybe happens once a month but not consistent so troubleshooting is hard to reproduce.

Other thoughts since most have resolved by a factory reset of the 2635-222?

Could be part of an errant program program you've forgotten about. 

If it's not part of any programs, remove and delete it from the isy. Factory reset it and leave it alone for a while. If it doesn't happen any more, then it was probably being triggered by something in the system.
