OK I'm stumped by this one. I thought my old PLM was slowly giving up the ghost as it wasn't reliably picking up local ON status from devices.  Other than that everything was fine. Anyway, I purchased a new USB PLM, swapped the daughterboard, and restored the PLM successfully. No unusual issues.

Anyway, I'm still having local status issues. So far I've identified several switches that fail to report a local ON while other devices in that scene do show correct status. It will always show correct status when turning it OFF locally. Querying it in the console updates the correct status as does turning on other lights in the scene. Scene tests all pass and I'm seeing 2-3 hops remaining on traffic.

Here's the kicker. When turning it on I noticed in the admin console that it DOES show ON for a brief second before showing OFF again. I've turned off Home Assistant and disabled any programs that might be affecting it. I monitored the program status and there were no programs that ran during my tests. As you can below "Office Recessed" was turned on locally at 10:53:44 and immediately shows off the same second.

Any ideas on how to proceed from here?

Update: Resolved?

- did a link compare, it showed some discrepancies. Did a restore of the device. No luck.

- did a factory reset of the device and then did a restore. No luck.

- removed device from ISY, factory reset device, re-added back to ISY. Success!

Hope the same holds true for others I identify.

Actually, when I think about it now, I have had half a dozen devices (not Insteon) in my lifetime, fail to reset properly, due to a failing power-on reset capacitor. Then you hold the power on button longer and away it goes the next time.

With Insteon's hardware history, it seems very likely that a cheaply made device, and with Insteon's bad capacitor reputation, why not just a 'luck of the draw' factory reset success story?

I guess the advice for some in the future is..

....just keep hitting it more factory reset until your finger wears out.

I've done 4-5 so far and that seemed to do the trick. There's more to fix but I'm burnt out on insteon for the day. 

My setup originally dates from the early days of the original 99i. Every switch in the house (50+) have been replaced at least once to move from v1 to v2 and through several PLMs and a couple of 99x's. I guess links could have gotten corrupted over the years. Who knows. I do feel my time probably would have been better spent migrating to Polisy then rebuilding PLM's  and re-adding them to a legacy controller.

I guess one could say you got in too early in the Insteon game and have been through the meat grinder or the 'bleeding edge'.
I got in with BSR, later becoming X10, but had a huge blank spot, missing the early Insteon single band era, so my experience with Insteon may have been more positive.
