Issues with lights turning on due to Alexa hunches error

It's not the first but the fifth time I had to disable a lamp device in Alexa app to stop her hunches to try to turn off a lamp in my house and then suddenly she (or something else) turn that lamp on instead.

When I search the Logs, I can only see the Insteon device location followed by a simple "Error" and the log type Log is followed by another log type -2.

When checking the isy.io eventlog, I found the following:

I've opened a support ticket and they told me Alexa sent the command correctly, but they told me Alexa sent the command correctly but "something" when wrong and Alexa canceled the action.

When I look into Alexa app for hunches, I can see that indeed it is not asking me if she did it correctly or not, but a Cancelled status. In cases like this, Amazon is telling me that the fault was due to the device (this case IO portal).

Does anyone experienced this issue?
