Notifications not working with programs

I've setup my notifications and I can run a text from the notifications tab and get a test email or text message. However, when I go to use the notifications in a program they dont' work. Even when I manually run the "then" statement. Is there anything that I need to do to setup notifications to work in programs? Any recommendations?

If you are using custom templates, then you HAVE TO make sure that neither the subject nor the body are blank.
