Insteon 2491T communication problem

I have a Venstar/Smarthome thermostat that I can no longer communicate with.  It's been in service for about 7 years.  My ISY reports that it cannot communicate with it.  I tried a reboot on the thermostat but no change.  I wasn't sure how to factory reset it so i held the set button while powering up.  Does anyone know how to factory reset?

I deleted it from ISY and when I added it back I got this error:

I wanted to try a factory reset before I order a replacement.  Thank you.
