Having Problems Connecting Leviton Switch

I'm trying to add a Leviton Switch and an Ecolink Z WAve Pir Motion Sensor...  My ISY994izw Finds the Ecolink Motion Sensor just fine, but refuses to find the Leviton Z Wave Switch.  They are within 2 Feet of each other.

 

The Ecolink is Battery operated, and the Leviton is Hard wired, so I'm not entirely sure where the failure is.

 

 

If you haven't already, try excluding the switch and then try adding it.

The switch has never appeared on the Universal Devices admin console.  When I put it into pairing mode, it fails to find it.  It CAN find the Switch that is sitting 2 feet above it, though.. lol

Does the leviton switch have a means to reset it to factory default settings?  I cannot help but suspect it may already think it is part of a zwave network.

 

I just happen to have a Leviton Z-Wave dimmer install sheet handy.

To exclude, hold the top paddle for 7 seconds. The LED should flash amber. Start exclude from the ISY and then tap the top paddle once. The led should flash green once.

To reset a Leviton Z-Wave dimmer to factory default hold the top of the paddle for 14 seconds. The LED should flash amber/red. Release the paddle and the dimmer/switch resets.

As to latest version, they would work on the 4.0+ release but not well. The dimmers were recognized and operable but the ISY did not recognize Hails so the ISY was not updated from on/off/dim actions at the switch.
