Can't add new Z-wave devices

I just attempted to add a Leviton DZPA-1 ON/OFF receptacle to my system and the system gets stuck adding a device to the Z-wave network.  I tried another device and I got the same thing.  I have other DZPA-1's in my network and I was able to add them with no trouble at all, so I know they are compatible. At this point it looks like I have lost the ability to add Z-wave devices.

I can control Z-wave devices so that would imply that the dongle is working.

I tried unplugging and re-plugging the Z-wave/Zigbee dongle and then restarting IoX, but it did not help. I'm running IoX 5.7.0.

Any suggestions?

Did you try excluding before trying to include? Try adding with the event log open and set to level3. It might give you a hint to what is going on.

There are many more "status request" lines, but no response from a Z-wave device. 

The DZPA1 was blinking amber, indicating that it was ready to be included.

Thoughts?

Did you press the button on the Leviton after was blinking?

I have a few of those and process I have in my notes is...

 Leviton: Include or Exclude Press hold till blink, Press again

I tried it with and without the extra tap.  No joy. 

In case it was the DZPA1 at fault, I tried to add a Minoston MP20Z and it failed too.

This may or may not be helpful, but I'm not sure the Minoston is a good test.  I have had a ton of problems adding Minoston devices unless they are really close to the dongle, both the Zmatter and the Zooz 700 series.
