You did power cycle the ISY994i and 2413. Letting the PLM fully boot first? We have seen strange things if the wall wart supply for the ISY994i has issues but this doesn't exactly fit the problems we have seen. I would suspect the 2413S has a problem communicating. How old is it and can you find the revision number on the back? Can you do a test to one of the problem devices. Showing the communications in Event Viewer on Level 3 and see what it shows. Or you suddenly have a noise maker on the power lines strong enough for the 2413S to think there is a communications on the line. Oh one more thought. Are the modules in question new enough the be Dual Band or the older power line only modules?

9 minutes was removed and changed to a maximum of 8 minutes. For many of the modules over the years. Though the reasoning for it I can't remember.

Yes. The old 2456S3 ApplianceLinc was a single band power line only module. I have see two major versions of them. V1.3 was the original ones and v4.? (seen a few revisions) a slightly better design. V1.2 was a recalled revision as switching certain loads would cause the magic smoke to come out. 😀

You may have an intermittent power line noise or signal sucker issue. The range extender or any other dual band module. Will receive the sensors RF signals and pass them on over the power lines back to the main setup. If the sensors RF signal is not getting back to the main setup alone. If it starts again. Try and make a note of what maybe running when the issue stares again.

It does sound like a range issue, interference, power line noise. As it worked in the office and could be added closer to the PLM. That is probably the device receiving and sending the sensors RF signals. You are having problems adding the sensor not trying to add the range extender. That I believe is not possible to add the the system. If you have an LED type light in the opener try temporarily removing it. I have seen report of some causing problems. If you unplug the opener itself. Can you then talk to the sensor? I have seen some openers electronics. Reported to cause issue and having to be on a FilterLinc to isolate it.

Another point. The I/OLinc besides being power line only. Also has a small switching power supply in it and the old Smarthome did the different sized capacitors trick. The early revision 1.0 has what I would call too low a sized capacitor in it and when the relay is on. The power supply voltage sages. That may or may not effect your setup. If the unit is older and has aged.

Yes they can fail. Most where probably the older designed I1 power line ones but I have seen some of the newer ones also being reported as going bad. Newer ones went to a switching power supply and may have had the capacitor issues we see in the 2413S PLM. I have also seen not working tact switches under KPL buttons reported. I have had some 2456S3 ApplinceLincs fail. One actually started pulsing On and Off at about a 60 cycle rate. Replacing the capacitors in it fixed it. My ISY994i also had a power supply wall wart fail and I did a SD Card swap for good measure.

Keypad died. Can I install a toggle and control the fanlinc via Alexa?
Could also depend on the KeyPadLinc's age. The early ones where also power line only.

The old ISY99 was mentioned in the question. So I can't remember when my moth balled ISY99 could do. Those are in my ISY994i drop down list.

You may want to check the drop down list in the Administrative Console of the ISY99i. As its last firmware may not have the later Insteon modules in its firmware. So it could be properly controlled. I also think an On Off relay type is the safe way to go. As dimmers frequently don't like any inductive motor loads. There is also a small time in the AC Cycle where there is no current to the motor at the zero crossing of the AC wave.

Isy 994i starting to die??
If trying a new power supply. Did not fix the issue. Checking the SD card and RJ45 jacks for bent or corroded contacts. May give you some added help.

Replace 2466DW With 2477D

Insteon Siren
I believe the Siren and older Alert Modules. Where two of the module. The Old Smarthome Crew. Didn't supply any programming data to UDI. So it was reverse engineering for both modules and I am not 100% sure it was totally controlled but that is my opinion and not official. I too am using the chime function not siren. It is really not a true Ding Dong for me. More like a cycle of high beeps and lower bops. Not what I would think being called a chime but Smarthme called it that!

Mine Chime is also Very Loud and I left the factory sound limiting sticker over the sound output port. Mine are also very early production and in my case. If I manually set anything with the side buttons. On a power interruption it powers back as Off.

On mine it had to be in a scene to set how it worked. In the main drop down list. I found setting the chime sounding time was not working until it was done in a scene.

The faint slow blink sounds like the modules power supply is failed and there is excessive AC on the DC supply. Sounds like a new module replacement.

All the modules in your system. Will presently have the USB PLM's six digit Insteon id in their link database's and will not respond to the six digit Insteon id of the serial PLM and probably would not respond to commands. Best to wait and properly change the PLM through the presently used controller.

Slow Blinking Lights?
Since the old style halogen bulbs flash. It maybe the module starting to fail. Some brand LED bulbs do flash on dimmer style modules and sometimes you had to choose different models to find some that would work. You may want to see that the connections to the module are good and solid. A power cycle may give you some information or a factory reset and reload the information to the module.

EISY Irrigation Controller
Have you compared the links in the module to the ones in the ISY Administrative console? Are you turning on and off each individuate sprinkler output. As only one can be on at a time. If you have set output 8 to optional pump control. Is it turning on when any other output is on? If the outputs are actually going on and off. Have you verified the 24VAC sprinkler valve transformer is working?

Unable to add isy portal module
After the ISY994i was discontinued. We where given a few months to purchase addon modules. Not sure about a portal modules but all the others are no longer available to be added. Also we had to use the My Account tab on the UDI web site to purchase modules in later years not the purchase module in the administrative console.
